What do I need to have on the day of my child’s dental appointment?
On the day of your scheduled pediatric dental checkup, a parent or legal guardian must be present with:
- Photo ID or Driver’s License
- Insurance Card
- Appropriate custody or legal guardianship paperwork (if applicable)
