Please print out the paperwork and fill in your child’s information, don’t forget to bring it with you to your appointment. If you are unable to complete your paperwork in advance, please arrive 20 minutes prior to your child’s appointment to allow time for completion.

What can I expect the day of my child’s appointment?

On the day of your scheduled appointment, a parent or legal guardian must be present with:

• Photo ID or Driver’s License
• Insurance Card
• Appropriate custody or legal guardianship paperwork (if applicable)

If a parent or legal guardian is not available to attend your child’s appointment, you must submit your New Patient Paperwork in advance and list other person(s) authorized to bring your child. The parent or legal guardian must be readily available on the day of the appointment in case the office needs to reach out via phone.
